Abstract

For the placement of a dental implant, it is a common problem that the too narrow width of bone to permit the total inclusion of an implant within the bony housing. These losses of the bucco-palatal (lingual) dimension of ridge led to the development of a technique that expands the narrow ridge. When the bucco-palatal (lingual) bone width is 3 mm or greater but <6 mm, augmentation of the ridge using a ridge splitting and expansion technique is a viable option. But, some cases in ridge splitting with vertical cut showed complications such as fracture of buccal bone plate, paraesthesia or prolonged pain. Therefore, in this case presentation will describe the cases of modification of ridge split and bone expansion osteotomy done without vertical osteotomy cuts. Two patients with narrow ridge were selected for implant installation with modified ridge splitting and expansion without vertical osteotomy cuts technique. In all cases, Modified ridge splitting and expansion without vertical cuts technique was done using blade, mallet, spreader (Ace Bone Spreader Osteotome Kit, Brockton, U.S.A) and ridge splitting kit (RS kit, Dentium, Seoul, Korea). With this technique, implants could be placed immediately within the expanded ridge and complications such as fracture of the buccal bone plate and prolonged pain, moreover, possibilities of additional guided bone regeneration could be reduced. Modified ridge splitting and expansion technique without vertical cuts could be used in the narrow ridge without any complications, therefore it could enable more functional and aesthetic implant placement.
